Composite Fillings

The choice between a filling and a crown depends mainly on how much healthy tooth structure remains after the dentist removes the damaged portion.

  • A filling restores a limited area of decay or damage while preserving the surrounding tooth.
  • A crown covers and supports the entire visible portion of the tooth when what remains is no longer strong enough to support a filling.

When a Filling May Be Sufficient

  • The decay or damage affects a small portion of the tooth
  • The surrounding structure is strong enough to support a bonded restoration
  • The tooth does not have extensive existing restorations

When We May Recommend a Crown in Framingham

  • A large area of the tooth is affected by decay, fracture, or a previous restoration.
  • The tooth has cracks that put the remaining structure at risk
  • The tooth takes heavy biting forces and needs full coverage for protection

Dr. Maddahi evaluates each tooth using clinical examination, digital x-rays, and intraoral images. When the recommendation isn't straightforward, he discusses your options and explains what each approach can accomplish over time. The goal is to restore and protect the tooth while removing as little healthy structure as possible.

We serve patients throughout the MetroWest area, including Natick, Sudbury, Wayland, Ashland, Marlborough, Southborough, Westborough, Hopkinton, and Wellesley.

Crowns and Bridges

Both options can give you a stable, functional replacement, but they differ in how they are supported, what they require from the surrounding teeth, and how the treatment unfolds.

Why a Bridge Might Be a Good Fit

  • Uses crowns on the neighboring teeth to support the replacement tooth between them
  • Treatment typically takes two to three visits over a few weeks
  • May be a practical choice when a neighboring tooth already needs a crown
  • Requires reshaping the supporting teeth, even if they are otherwise healthy

Why Consider a Dental Implant

  • Replaces the tooth independently without involving the adjacent teeth
  • Requires a surgical phase and several months of healing before the final restoration
  • May help preserve bone in the area where the tooth was lost
  • Candidacy depends on bone volume, gum health, and your overall oral health

Neither option is universally better. Dr. Maddahi evaluates the entire area, explains the trade-offs, and recommends the option that fits the clinical findings and your priorities.

Dental Implants

Insufficient bone at the implant site does not automatically rule out treatment. In many cases, a bone graft can rebuild enough bone to support an implant, but the decision depends on a careful evaluation of the specific site.

How Dr. Maddahi Assesses Bone Loss

Dr. Maddahi performs a clinical examination and takes 3D CBCT scans to evaluate the height, width, and quality of the available bone.

The cause of bone loss, whether from infection, gum disease, trauma, or prolonged tooth absence, factors into the treatment approach

Imaging helps determine whether grafting can create adequate support or whether an alternative replacement is more appropriate.

What Bone Grafting Involves in Framingham

Dr. Maddahi places bone material—which may be synthetic or taken from you or a donor—at the deficient site to encourage new bone growth.

Healing takes place over the next several months as the graft forms a scaffold on which new bone can generate. Dr. Maddahi will reassess the situation after healing to determine if the graft has been successful and if we can proceed with implant placement.

Are You a Candidate for Bone Grafting?

If you have been told you lack sufficient bone for an implant, or if you lost a tooth some time ago and are now considering replacement, a thorough evaluation can clarify your options. Call Orris Dental in Framingham at (508) 270-6770 or book an appointment online.

Family Dentistry

The most helpful thing you can do as a parent is to keep the conversation simple and positive. Talk about the visit in matter-of-fact terms. You might say the dentist will count their teeth and make sure everything looks healthy.

How to Speak About the Visit

Try not to share your own difficult dental experiences, as children pick up on those feelings quickly. If your child asks what will happen, short and honest answers work best.

You can explain that someone may clean their teeth with a special toothbrush. Avoid words like "hurt," "shot," or "drill," even to reassure them it won't happen. Children tend to focus on the scary word rather than the reassurance around it.

What to Share with the Dental Team

If your child has habits like thumb-sucking or grinding, or if you've noticed concerns about their sleep or breathing, let the team know ahead of time. This helps Dr. Maddahi look for related signs during the visit.

During the Appointment

Let the dental team lead by introducing instruments and explaining what they'll do. Some children take to the dentist right away. Others need a few visits before they feel fully comfortable, and that's completely normal. Early visits build familiarity, not a perfect appointment.

Schedule Your Child's First Dental Visit

HealthyStart® System

HealthyStart and orthodontic treatments such as braces and clear aligners serve different purposes and are used at different stages of your child's development.

Early Intervention During Growth

  • Addresses oral habits such as thumb sucking or tongue thrust
  • Supports developing jaw and bite relationships during growth
  • Guides erupting teeth toward more favorable positioning

Orthodontic Tooth Movement

  • Moves teeth that have already erupted into corrected positions
  • Typically recommended after most or all permanent teeth are present
  • Includes options such as braces and clear aligners

HealthyStart may create more favorable conditions for the teeth and jaws, but early intervention does not guarantee that braces or aligners will be unnecessary. Some children benefit from interceptive treatment followed by a shorter or simpler course of orthodontic care, while others may still need full treatment as they mature.

Individualized Recommendations

Each child is unique. Dr. Maddahi can evaluate your child's development and explain whether HealthyStart, later orthodontic care, or continued monitoring is the most appropriate step.

To learn more about the benefits of early intervention with HealthyStart, call Orris Dental in Framingham at (508) 270-6770 or book an appointment online.

Invisalign® Clear Aligners

Teeth aren't permanently fixed in one position after orthodontic treatment. Bone and supporting tissue that surround the teeth need time to stabilize, because natural forces can cause teeth to shift throughout life. Without a retainer to hold them in place, even well-aligned teeth may gradually move back toward their original positions.

What Orthodontic Retention Involves

Retention is a separate stage of Invisalign treatment, not an optional step. Here is what you might expect:

  • Some patients initially wear a removable retainer full time before transitioning to nighttime wear only
  • Wear schedules vary by patient, so follow Dr. Maddahi's specific instructions
  • Retainers require regular cleaning and occasional replacement
  • Inconsistent retainer wear can allow teeth to shift back toward their original positions

Think of retention as protecting the time and financial investment you put into treatment. Dr. Maddahi recommends the right retainer type and wear schedule for your case and will let you know when adjustments or replacements are needed.

Non-Surgical Periodontal Treatment

Gum disease begins as gingivitis, an inflammation of the gums caused by bacterial plaque building up along the gumline. Gingivitis is typically reversible with improved oral hygiene and professional care, and it does not inevitably progress.

However, when inflammation persists and risk factors are present, it can develop into periodontitis, which affects the bone and connective tissue that support your teeth.

Key Risk Factors for Gum Disease

  • Inconsistent brushing and interdental cleaning
  • Tobacco use
  • Diabetes
  • Genetic susceptibility
  • Certain medications that affect gum tissue
  • Infrequent professional dental care

Reducing Your Risk

  • Brush twice daily and clean between your teeth with floss or interdental brushes.
  • Schedule regular professional examinations and cleanings
  • Avoid tobacco products
  • Manage diabetes and other systemic conditions
  • Seek treatment promptly if your gums bleed, swell, or recede

We can help control periodontitis and preserve your remaining support; however, bone and support that have already been lost cannot simply be restored. Early intervention offers the best opportunity to keep the disease from advancing.

Sleep Apnea and Snoring Treatment

Continuous Positive Airway Pressure (CPAP) is an effective treatment for moderate to severe obstructive sleep apnea. Unfortunately, some patients find it difficult to use consistently. Common concerns include mask discomfort, skin irritation, unintentionally removing the mask during sleep, difficulty falling asleep with the equipment, and challenges transporting the machine while traveling.

Call Your Sleep Doctor First

If you are struggling with CPAP, the first step is to speak with your prescribing sleep physician. Adjustments to the mask style, air pressure, or humidification settings can sometimes resolve the issue. If CPAP remains difficult to tolerate, your physician may recommend evaluation for an alternative approach.

Oral appliance therapy may be an option for some patients, depending on their diagnosis, sleep apnea severity, oral health, and jaw function. Dr. Maddahi evaluates dental candidacy, creates and adjusts the custom appliance, and monitors the teeth and jaw throughout treatment.

Coordinated Care for Effective Results

Coordination between Orris Dental and the sleep physician remains essential. The goal is to confirm that the selected treatment is effectively managing the diagnosed condition.

Obstructive sleep apnea is a serious medical condition, but you may have options. Call Orris Dental in Framingham at (508) 270-6770 or book online to learn more. We also serve patients in Natick, Sudbury, Wayland, Ashland, Marlborough, Southborough, Westborough, Hopkinton, and Wellesley.

TMJ/Bruxism Treatment

In many cases, yes. Research has established a significant connection between sleep-disordered breathing and nocturnal (nighttime) bruxism.

What's the Connection?

When the airway narrows or becomes partially obstructed during sleep, the body may respond by clenching or grinding the jaw in an unconscious effort to reopen the air passage. For some patients, grinding is not a standalone habit but a symptom of an underlying breathing issue that may benefit from its own course of treatment.

Symptoms that suggest a connection include:

  • Grinding that persists despite consistent nightguard use
  • Daytime fatigue, morning headaches, or unrefreshing sleep alongside bruxism symptoms
  • A partner who reports snoring, gasping, or pauses in breathing

When Dr. Maddahi identifies wear patterns or symptoms suggesting sleep-related grinding, he may recommend a formal sleep evaluation to determine whether a breathing disorder contributes. Treating the airway issue often reduces grinding frequency and intensity, sometimes significantly.

Book an Evaluation with Dr. Maddahi

If jaw pain, worn teeth, or chronic headaches coincide with poor sleep quality, a comprehensive evaluation can help determine whether the two are connected. Call Orris Dental in Framingham to schedule an appointment.
